Grandma has CKD Stage 4/5, Heart Failure. Keeps on sleeping, not really eating. How much time do we have?

94F, UK, Congestive Heart Failure, Chronic Kidney Disease Stage 4/5.

It looks as if my Grandma is coming to the end of her life. She has congestive heart failure and chronic kidney disease. Her situation rapidly worsened over the last 3 weeks resulting in 4 hospital visits (for falls mainly).

Yesterday she experienced hypoglycemia (she is not a diabetic), her reading was 0.9. They got her back but she's been very disorientated. She is prone to falling and struggles to recall conversations.

She isn't talking much but wants to just sleep. She is eating very little and producing a small amount of urine in a day. She's quite literally sleeping 22hrs a day.

She is being kept comfortable and can walk very little distances in a day (perhaps 4-5m?).

How much time do we realistically have and what will the end look like? I am told heart failure folks die in their sleep, is that true?

She has children across the UK/Europe and we want to be able to give them the opportunity to be with her but we're really not sure.

Thank you.
