Advice on how to approach guy leaving condom inside

I need help on how to handle this. Never had this happen to me before. I had sex with someone I’ve known for 2 years. We fell off but reacquainted recently. He took me on a date & we ended up having sex. I was having cramp like pain on and off on day 6, 7 and 8. It felt like a really bad period cramp. I never take pain meds but this pain wasn’t feeling right. 8 days later I felt something weird down there. I pulled and it was a condom!!! I felt so disgusted, nauseated, and disappointed. How do I go about this? Should I confront him? What do I say? He wants me as a girlfriend but after that encounter I’m turned off. We had a few drinks before sex, but surely he could still feel that the condom wasn’t on him right? I’ve also started spotting after taking out the condom. It could be a coincidence. I’m on birth control and wasn’t expecting a period anytime soon. We have a gym membership together. That’s the only attachment. He said he wanted to take care of me. But I keep thinking this is how you take care of yours? I’m definitely turned off from sex. I don’t know how to go about this. Please help.
