PS4 controller USB to PC causing major stuttering, audio loss, and loss of controller input. HELP!

I've seen this exact problem posted by people playing on PC, whether it be steam or otherwise and it just started happening to me. The game stutters frequently and loses audio intermittently. Unplugging my controller via usb then plugging it back in fixes it for a very short period.

Has anyone ever found a permanent fix for this? I know I can't be the only one suffering through it right now. I basically can't play until it's fixed because it's way too frustrating.
