DIY, What is wrong with this water pipe/foundation? Leaks... (cross post r/fixit)

At first it only happened when it rained (or at least it started on a rainy day), but it hasn't been raining that much lately and I'm still soaking up a couple towels a day. It isn't obvious that it's coming from the water valve, but it seems like too much of a coincidence for it not to be? The ground outside of this part of the house has a pretty good slope. Any thoughts on how to go about diagnosing/fixing this? You can see some bubbles in the photos where the paint on the concrete has come up.
