You start with Polyphemus and just play a normal game. No Brimstone and no Fetuses. All rooms and drops allowed,

As NL is all ways saying what a great base Polyphemus is for other items, why not start with it and see what goes well with it as many items in combo with Polyphemus have surprising effects.
