What is the most intense/ extreme / dangerous kind of play you have witnessed?

Last night as I was watching a friend preform auto-erotic asphyxiation while self suspended for the 4th or 5th time(total not in one night) I realized that I've seen quite a bit of edgeplay and things that many people never get to see. I kind of wanted to ask what kind of dangerous play do you see regularly but felt it was too limiting.

I've also seen two lynchings, a lot of breath play, a friend get beat so bad it made people had to leave the room and even one leave the party, saline injected breasts, intense floggings, someone get 350+ needles in his back(only saw the end result didn't watch), some fairly heavy humiliation scenes one of which included food and a garbage can, and that's all I can remember right now but I feel like I'm forgetting a big one.

Note: When I saw "dangerous" I do NOT mean unsafe. I mean edgeplay/risky type of play.

So what have you guys seen?

Edit: I'll add this one cause I can. Last night I did a scene in a gas mask that had a tube and near the end my top said she found a spider and put it in. I of course didn't believe her until I felt something move on my face. But even then I knew it wasn't a spider she wouldn't do that and I knew there was a several box elder bugs around the play space. After I got out nearly everyone said they would have freaked out even the more "hardcore players" honestly before the scene I don't know what I would have said to it but I just went stonewalled and while it wasn't exactly fun it wasn't bad.
