What exactly constitutes sexual assault/harassment?

Not sure if this is the subreddit to go to for this, so direct me to the right one if I'm misguided here.

Situation 1: Let's say the girl has sex with a guy where she was too drunk to remember consenting, but she has hooked up with him (never had sex with him though) in the past.

Situation 2: No sex, but sexual things happen, like clothes coming off and so on. The girl was black out drunk, and the guy, a friend of hers, was warned, "Don't do anything with her, you're sober and she's completely gone." She doesn't remember initiating, he says she did initiate.

Situation 3: The girl told the guy not to have sex with her if he doesn't have a condom, and that she doesn't want sex. He puts it in without a condom, takes it out when she yells at him.

Are any of these situations sexual assault or harassment?

EDIT: no one will call the police/go to court over any of these, just to clarify
