LPT Request: How to reduce appearance of scars?

I have a patch of scars on my shoulder. I have fairly light skin and most of the scars are discolored and slightly raised. I know makeup works temporarily but is there anything else? I've currently been using Palmer's Cocoa Butter but I'm not noticing a change.
