I am really sick of chronic refusers and having to document on them..

Rant

I have been experiencing an upsurge of patients like these at the SNF that I work at over the last 6 months to a year. The ones that are fully alert, fully orientated, that seem to refuse everything that you want to give to them. When asked "why" they just shout and curse at you in response instead. But even when they ask about every med that you try to give them they reply very aggressively along the lines of "I don't have that" and "You're lying". Those meds are also something that they have something that they VERY much need, like acu checks or divalproex. I am just tired of all the notes that I have to write saying that they refused their meds and treatments, and worse if I have to get Psych and/or the family involved. What blows my mind is that they float some patients from other units to me and the patients do that kind of behavior, and then I look back at their progress notes and I see that it has been MONTHS of the same behavior documented by the nurses from that other unit. I just wonder why the hell they are still here. Maybe it is because of money and/or because those people just wanted three meals and bed and there's here just to take up space and waste everyone's time. I dunno we have been seeing so many homeless patients recently as well. What kind of inspired me to make this post because a recent re-admit that was floated to my unit from another unit is doing the same thing but on a more aggressive scale. Everyday he gets pissed off at me waking him up to do meds and treatments, particularly acu checks, and then he even gets pissed off at me for doing it while he is awake, all the while calling me "disrespectful" and "selfish" for daring to interact with him at all. Today he told me to wait after he finished dressing to do his acu check, and then when he finished he just straight up ignored me and then told me to document refusal. Huge ass waste of my time. That person had months of charting on the same exact behavior, and he even got sent to the hospital twice but still managed to end up back here and doing the same exact thing.

Ridiculous.
