Help me (f20) revise my ultimatum for my girlfriend (f25)

Hi Reddit, this is kinda pathetic but I have no close friends and I need a third party to look at my ultimatum to my girlfriend. I don't want to be harsh, but all of this needs to be said. I love her a lot, but if she yells at me again, I'm breaking up with her. My ultimatum is the following:

I want to have a sincere and open conversation about our relationship. There are several issues that have been causing me distress, and I need you to understand how they affect me.

First, I want to address the situation regarding the lipstick. When I wore that lime crime lipstick, you gave me an ultimatum, and I immediately complied without hesitation. I didn't have to think twice. Yet, when I've asked you countless times not to yell at me or become frustrated over trivial matters such as misunderstandings, it feels like my pleas have gone unheard. Most people would have left a relationship after the first instance of being yelled at. Kiana, I love you deeply, so I've tried to make excuses for you but I'm coming to my wits end.

I took care of you to the best of my ability for months after your first surgery, and I hope you feel that way too; but when I needed your support due to a sprained ankle, a simple request to find my glasses, it felt like you couldn't handle it. I rarely ask you for help, and it's not easy for me to do so. You know this about me. The one time I needed you in the same capacity you needed me, you broke down. Despite all the reasons/explanations you've given for this behavior, I can't continue to accept it. You have to agree it's not fair to me at all. It's hard when I keep trying to meet your relentless negativity and bad demeanor towards me with the love I know you deserve.

Here's my ultimatum:

  1. If you ever yell at me or scold me like that again, I'll need a break. A actual break where I'll stay at my dad's, taking my belongings with me, and leaving you in the condo.

  2. I shouldn't have to ask you for help with cleaning or for basic tasks. You're a grown woman, and it's time for you to take responsibility for this. I'm the only one between the two of us who consistently tries to keep our space clean, and I can't keep up when you leave your belongings on the dining table and bedroom floor all the time. What's more, PLEASE don't offer me things or help if you're going to complain about it afterward. Why offer it to me in the first place? Your lack of effort is disheartening, and it's time for a change.

  3. I've seen how messy your old bedroom was when we first started dating, and I assumed it was due to the quarantine. However, it's been two years now, and I'm disappointed that you only clean when I ask you to or when we argue. I can't envision a future where we raise a child or build a life together if this pattern continues.

  4. I shouldn't have to beg for affection or initiate physical contact. Our conversations about this issue have been numerous, but I want you to greet me with a hug and a kiss when I come home, just as I do for you. You have also broken your rule about you initiating at least twice a week a month ago now. It's essential for me to feel loved and valued.

  5. We began going to the gym together, but you really fought me on it and it was honestly exhausting for me. It’s been six months. It's frustrating that I had to persuade you, and you dismissed my requests for help with meal prep by requesting we get pizza for four days in a row. Even after I told you I was sick of it on the fourth day, you insisted we get pizza because of your appetite. This behavior makes me feel like you don't want to support me in my goals.

I don't want to give up on us, the time we've spent together, and the future we could have, but I'm exhausted. I'm not your girlfriend; I'm your special friend that you live with and cleans up after. My ultimatum is simple: treat me with the love and respect you show your friends. Stop lashing out at me over minor issues, and start treating me as your partner should. I hope you understand the gravity of this situation and that we can work together to improve our relationship.

TLDR; I've given an ultimatum to my partner. They need to treat me with respect, stop yelling over trivial issues, take responsibility for cleaning, keep promises, and show affection. If they can't meet these basic expectations, I'll take a break from the relationship. I'm exhausted and need them to treat me with the love and respect they show their friends.
