Expropriation

Question/Landlord

Ran into an odd situation, my mother owns a house that's pretty old and she has rented it out to someone who used to work for my father (since passed) and has given him one hell of a deal for quite a number of years. He's on a month to month lease and there has been very little in terms of formal paperwork ever done.

Last year the city has decided they are expanding the road and has informed her that they are expropriating the home. She informed the tenant that this will happen in the future and she has no control over it, which is true. The city has given her an offer (surprisingly quite fair) and has given her notice and told her the home must be vacant. She has given the tenant written notice (90 or 120 days, not sure) and the last month or two being free rent. She is trying to be as reasonable and fair as possible without being taken advantage of.

She had no interest in selling the house but she's not overly upset as at her advanced age it is a headache she doesn't need and the money will come in handy as the rent barely covered the costs anyway. I was looking for the proper form on the LTB website but I could not find one that covered this situation. It's not exactly an N12, but she needs to do this properly if it isn't going to (potentially) turn into a circus.

What form would you use for eviction due to government expropriation?
