how do i anonymously contact the health department for food service violations?

there’s a few things at work our manager just refuses to care about, and i’m hoping i can anonymously report the things happening to at least scare management into fixing/considering these issues. there’s so many sites i’ve looked at and none seem to be fit for simple food service complaints, some threaten fines if the problem isn’t serious enough?

to give some insight- we’ve baked various kinds of bugs into cookies, have a growing drain/fruit fly infestation in the back of the store (right next to our ingredients, many of which are open wide?), our manager vapes indoors right next to the ingredients we use, we’ve baked metal shards into cookies as well, many of my coworkers eat our product where we prepare it (and as we prepare it…), we have sold product our corporate considers expired (week old custard, yum!), we have sold product that has come into contact with mold and were told to pick out the mold and sell the rest, many coworkers leave cleaning chemicals on the counters where we prepare food, many coworkers don’t have food handlers permits and brag about getting away with it, and our owner has let his kids run around touching things (not super serious but still ick..).

i just can’t for the life of me figure out where to go with this. any help would be appreciated!
