I absolutely adored this game way back when. However, the predatory microtransaction system, more or less being abandoned by the developers and the fact that it has been slowly dying for at least 5 years kind of leaves a sour taste, to say the least.
Is there anything, anything at all, like it out there? I suspect not really, but what games come closest?

Wait for the rework in december
There's been a few reworks over the years if I'm not mistaken, but afaik none of them have really changed that much all things considered?
Idk i Quitted the game months ago and i will plsy it again with a friend after the rework
I've never played anything else like rotmg. I sucked at it lol so I didn't stay long but yeah I over my years of gaming, do not have anything I could think to recommend that would scratch the itch for that.
It really is a unique little game.
Other games have hardcore modes, but no mmo I've seen is all perma death like rotmg. Plus it being bullet hell on an mmo where you already at mercy of ping and shit. If you don't care about the mmo aspect I would look into stuff like risk of rain 2, dead cells, enter the gungeon, rogue legacy pretty much why rogue like/lite.

Haven't found a game really similar, unfortunately.
There's Darza's Dominion! Steam reviews aren't particularly hope-inspiring, but its downsides might be less awful than ROTMG's at the very least.

You can try some single player or coop roguelites/bullet hell games. My personal favorites are Noita, Hades, Risk of Rain 2, Enter the Gungeon, Cuphead and Returnal
