Looking for walk-in clinic recommendations? need someone who will fill out paperwork

I don’t have a family doctor and I had a confusing/not so good experience at the walk-in at superstore.

Looking for a Dr who would help me fill out some paperwork so i could get some accommodations at work due to my illness.
