Can I appeal a “substance use disorder” diagnosis in my medical chart?

I am from Minnesota. when I was in my teens, I struggled with mental health and was hospitalized several times. I expirimented with drugs and my relationship to alcohol was bad.

some doctor, at some point, decided that every drug I’d admitted to having used once or twice was a disorder.

this means my chart tells every doctor i’ve seen that I have amphetamine, benzodiazepine, and LSD use disorders

amphetamine sounds bad, but it was MDMA and some kids adderall prescription that I had toyed around with like THREE times.

I mean I literally only did any of those drugs once or twice in my teens, and I have abstained from all drugs for many years.

can I somehow appeal this or is it permanently in my chart?
