When did Doctor Who start numbering the Doctors?

Seems like a simple question enough, but I can't find a definitive answer anywhere.

Obviously, nowadays it seems obvious to give each incarnation of the Doctor a number, but when did this first start? Was it as soon as Troughton took over or did they not believe regeneration would be a regular fixture of the show until Pertwee?
