Help with Birth Certificate Correction

Not sure if this is the right sub, but kinda need help lang. So I was applying for my passport, but DFA didn't process since they noticed na clinic name lang yung nasa BC ko, and wala yung address itself.

The lady just told me na I need to go to Manila City Hall to have it corrected but I wasn't really given the requirements that I need to have it corrected.

I was told by someone working in a different city na requirements daw should be; baptismal certificate, supplemental affidavit, and copy of IDs. I'm just not sure if the same applies sa Manila City Hall.

We already tried calling and sending an email to Manila City Hall, but no response from them until now.

PS I have always thought that nothings wrong with my BC. For my 20+ years of existence ngayon lang may nag call out. 🫠
