Looking for a movie, only have a rough description.

There is a guy whose looks get distorted to look hideous in the beginning somehow, and his wife and daughter die (or so he thinks) but this lady is leading him to join this group of people with photographs permanently floating in front of their faces that make them look pretty. At the end of the movie Incoming Spoiler the last step of joining this group is sacrificing this lady that has been leading him through this process, but halfway through he realizes that it was actually his wife the whole time, there is a fight scene and they escape. sorry if my description is too vague, but it has been a while since i have seen it.
