Barefoot shoes with hammertoe = lots of pain!

I switched to five finger shoes about 7+ months ago. I love them because I have really wide feet and most shoes make my toes go numb. But I've had a lot of pain which resembled tendonitis for over 5-6 months now. After a month of the pain, I decided to take it easy on the 24-30" box jumps to see if it would help. No luck. Then stopped sprinting. No luck. Then went to lower weight. No luck. Still pain. After going to the podiatrist this week, the conclusion is no stress fracture, it's hammer toe and a Bunyan on the outside of my foot inflamed due to my minimalist shoes. Any thoughts or similar situations with anyone else out there? They want to give me some shoe insoles, cortisone shot and possible surgery much later on as they get more cricked.
