Mental health, only been in 11 months, what should I do

when I got back from leave in June, I asked my lpo about separating due to failure to adapt because of stress, pay, and mental health, living conditions, etc. She wasn’t able to talk to me so she sent me to talk to my chief and a second class. As soon as the meeting started, my chief acted like he wasn’t interested in what I had to say, just had a “you signed a contract, so get it done” kind of attitude. My second class intervened and ask what the real reason of me wanting to get out was and said that he noticed he hadn’t been myself since I get back from leave and that I was “In the red.” We talked about me talking to someone whether it be chaps or bh. I got an appt to get a mental health referral (which was approved on July 8) I still haven’t been able to get an appt and have been getting the run around from mental health about forms I need to fill out, which doesn’t even apply to me, I’m just not sure what to do at this point. I tried the number you’re able to text but it’s no help because all they’ll really ask is if I want to go to the hospital. My mental health is deteriorating more and more as the days go on, I just want to get out. I don’t want to get kicked out with a dishonorable discharge for doing something stupid, but they’re making it hard.
