Screw Pile Caps Exposed

Should I be concerned that my screw pile caps are exposed? If so, what is the best way to patch the concrete slab-on-grade around the pile caps?

There is now some daylighting between the slab-on-grade and the walls of the unheated crawlspace; will spray foam insulation be effective in sealing it up?

Background: The previous owner constructed a sunroom addition with an unheated crawl space below. The concrete slab-on-grade that was encasing the pile caps has deteriorated, leaving them exposed. On the other side of this wall is an unheated crawl space. The slab has settled which has created a gap between the slab and the exterior wall.

I am l0ooking for some recommendations on the best way to fix these issues. Thanks!
