How to fill out V62 form?

Bought second hand car a year ago, unsure if ever got a logbook or if I’ve actually lost it. I can see on the car details online that one was issued when I got the car. Since buying I’ve changed address. I still have the new keepers slip. On the V62 do I still have to pay or can I send the green slip and get it done for free. Would I select yes or no for ever having a V5C in my name, and do I put I never got one or it was lost for the other section? Thanks
