Spearhead temptation dice question

It says if your opponent accepts the die, you get d6 DPs or if they refuse, the unit in question takes d3 mortal wounds.

When do these effects happen? Say opponent has one model swinging, misses one and hits one. You offer a TD. Opp declines and the d3 MW kills the model. Would the remaining attack still resolve and when would the model be removed?

If the opp accepted the die, you get enough to push your table to the ward 5+. Would that be available right then for the attacks in question?
