Frequent ear infections and want to know how to talk about it with primary care provider

I 22M have frequent ear infections that I believe is due to scarring which has left my eardrum with a permanent hole. I’d like to have some idea on what to talk about with my pcp about options for fixing this issue. My main concern is that I have to take an antibiotic regimen 3-4 times a year because of these infections. What are my options for curbing these infections? Is it due to the gaping hole in my eardrum?
