anyone still getting hit in the middle of their perfect Dodge?

I'm trying to fight the level 80 hologram Crownless and I'm slowly realizing that in the middle of my perfect Dodges I'm still getting hit by some attacks. am I dodging wrong or can some attacks just go through perfect dodges?
