How do players not constantly get their hands stepped on?

Especially during fumble recoveries. Their hands are all over the place on the ground where it would not be hard to get your hand stepped on accidentally (or on purpose), but I never see people grabbing their hands in pain or see injury reports for hand injuries. Taking a cleat to the hand HAS to fucking hurt. Does it happen and they just power through it?
