Bloodied Build: Insta-death when changing perk cards

I was changing around my perk cards the other day to add Hard Bargain, etc. to get caps and I died instantly.

I initially thought that it was just the “stub your toe” damage that happens every so often out of the blue. But it’s happened to me a few times.

A friend of mine was changing their loadout and instant death. They tried it a second time, same thing.

Anyone else experiencing this? Is it a known bug, but I just missed other posts about it?

I’m on the PS by the way.
