Adding partner to my claim what will he need to submit ??

Hi so I currently have a single claim on uc and wanting to add my partner as a joint claim? I’m aware by reading I have to do a change of circumstance and add him that way is that correct ??? Also what do they ask when I add him. What will he need to submit etc ????
