Intubated and sedated for 1+ week because of unmanageable delirium!
My husband (38M) was admitted to hospital after a motorcycle accident at the end of Feb. Injuries weren't too bad, no broken bones, pretty bad gravel rash over a lot of his body and some lacerations that required repair in theatre. He was due to be discharged on day 7 of an uneventful stay on a general ward where he was on a lot of pain meds. On day 6 he suddenly developed extreme paranoia and delusions that the nurses were torturing him and he was waiting to be killed. It escalated dramatically over the day while I was there with him, overnight it got worse and police were called when he was threatening self harm and had stolen scissors. I should mention my husband is very big and muscular and a veteran with severe PTSD that over the month of February was pretty bad and he absolutely hates hospitals so even though he was dealing very well while there it would have been distressing being there for almost a week. Because of his military history and size he was not able to overpowered, police were called and by morning they decided to intubate him to let him "rest" for a day or two. The night previous he did not sleep and he had apparently had maxed out all doses of other sedatives and this was their only option as by morning he was non-communicative and looked almost like he was in a trance, not responding to doctors, nurses or me when they attempted to have me talk to him.
After 3 days intubated he began waking and as he was demanding on a whiteboard for the tube to be removed, they did and he was still delusional. This was done hastily and without a plan, I could tell the doctor just panicked and had it removed. Within 10 minutes the tube was back in as he was attempting to leave and still delusional. By day 7 they came up with a plan that involved waking him in a quiet, controlled area. They believed it was delusions due to withdrawal of Benzos which he has been using excessively for quite some time and stopped during his hospital stay. I should also highlight he is a heavy user of cannabis as he is prescribed it for his PTSD and has been using THC on and off for about 4 years. Their plan seemed to work as they called me in and he was awake, not delusional and so upset, just saying how much he loved me and our kids. Within half an hour the delusions were back and he was not able to be controlled due to his aggressive demeanour and size and he was once again intubated, much to my horror.
That was on Thursday and since then they have done an MRI, spinal tap and I believe they are doing an EEG. MRI was normal and they said spinal tap will take a few days for results. If results are normal they plan on transferring him to a hospital with a mental health ICU where they will try to work out if these delusions are due to mental health.
Does this treatment seem right to you? It feels wrong and is incredibly distressing to see him intubated like this, especially as this ordeal has now been going on for a week and a half. He's never had delusions before and I'm afraid that he may just have a hospital induced delirium in which case this whole ICU setting would be making the situation even worse. His professional life is in jeopardy at the moment as he runs his own business and I am struggling to say the very least to keep that at bay and look after our toddler and baby. Any advice would be appreciated as I feel so helpless and lost and want to advocate the best that I can for him. thank you.
