advice on applying to UofT accelerated nursing program

Question

I am currently in my second year and have begun taking the required pre reqs for the accelerated nursing program, however i did quite poorly in my first year and not much better this year, my hope is that if i do well enough next year and find some good extracurriculars I may have a chance at getting into the program. I am confused as to whether the pre requisites will count as part of the calculated GPA or whether they will strictly look at my last 5 credits. And also with a lot of acceptences being offered to mature students do undergraduates have a chance? Any advice on how I can raise my chances of getting in or what other programs/routes I could try, specifically looking at UofT because they only look at last 5 credits where as most other programs look at last 2 years or cumulative gpa.
