Someone so insecure and self involved they literally cannot see outside of their own world or how they affect others, only how others affect them.

not egocentric (give too much credit to the person) and self centered doesn’t hit hard enough. This person would also take narcissism as a positive quality so that doesn’t work. implied insecurity is key
