How do you accent notes during a fast double stroke roll?

Pardon the newbie question here, but if someone is say playing a fast roll on the snare double strokes, and accenting some notes, are they accenting the first of a double, or turning it into a single?

Is it:
rrllRrllrrllRrll
(keeping doubles and just accenting the first of one of them)

or:
rrllRllrrllRll
(turning the accented R stroke into a single instead of double)
