Sick Leave Request

Just trying to get some advice, so my return to office date is May 5th. I was hired as a remote employee and due to Mental conditions will not be returning to the office. I have submitted a RA request and still waiting. I was thinking of asking to take 1 month of sick leave to get my self back together from the mental stress all this has put me in. My question has anyone done anything like this before, and if so what information did they request.
