Trying To Make Sure I Do The Right Things To Qualify For TPD

I have Bipolar disorder and have not been able to work in the last 3 years. My condition seems to only get worse as I age. I've made an appointment with my psychiatrist to fill out the TPD paperwork on the 25th of April. She is a licensed, board certified psychiatric nurse practitioner (PMHNP-BC). Is she a qualifying medical professional who can fill out the paperwork? Is she supposed to send medical records? Does she mail the paperwork in, or do I mail it? After sending in the paperwork, how do they follow up with me? Is there a specific phone number I should be on the lookout for? I am thankful for any help from you guys!
