Where are the universities in medieval settings?

Medieval universities were some of the most important institutions of the high middle ages that a lot of DnD is based but they're almost always absent unless the setting is more based on the modern world. Why do you think this is? Is this like the Tiffany Effect where something is historically accurate but feels unrealistic or is it something else?
