Why Some People Lose Weight Fast on Semaglutide—and Others Don’t (Yet)

Some people drop weight quickly on GLP-1 receptor agonists like semaglutide, while others don’t see major changes until 3–6 months in. A key reason might be underlying metabolic health—especially insulin resistance—plays a big role in how fast your body responds.

  • Early responders tend to have better baseline metabolic function. They lose weight quickly because their bodies are already primed to respond to appetite reduction.
  • Delayed responders often have insulin resistance or type 2 diabetes. For them, semaglutide first works to repair metabolic dysfunction—lowering fasting insulin, improving insulin sensitivity, and stabilizing blood sugar—before the weight starts to drop.
  • By 6 months, many delayed responders catch up. So slow weight loss early on doesn’t mean failure—it might reflect important metabolic changes happening under the surface.

Other accelerators?
Hormone replacement therapy (HRT) in postmenopausal women enhances weight loss response—likely due to improved insulin sensitivity and fat distribution.
Correcting thyroid issues, PCOS, or low testosterone may also improve outcomes.

This is one theory that may help explain the difference between slow responders and super responders. Of course, there are many factors at play in human biology—hormones, lifestyle, genetics, epigenetics, medication interactions, and pharmacogenomics—all of which contribute to how uniquely each of us responds to any peptide or pharmaceutical intervention.

My hope is to encourage anyone not seeing an immediate response to stay the course. You may be addressing and repairing deeper, underlying imbalances—and that process takes time. You are not broken!
