Help with application for Schengen Visa for Switzerland at SF Consulate

Hi

Plan - My husband and i plan to travel to Switzerland and Italy (equal days, entering via Switzerland). I am filling out form for Switzerland and have some questions -

1. In the travel plan, should I add just Switzerland as main destination or both?

2. Number of entries should be 1 or 2?

3. Number stays should be specific to Switzerland or the entire trip including Italy?
