Perfect Sire Achievement?

I think I'm just stupid, but I can't seem to get the Perfect Sire thing to work. I'm not taking pool, tentacle, or explosion damage. What's the "appropriate prayer?" I have melee on through phase 2, then tried both melee and range in phase 3. Is that my problem or am I taking pool damage without realizing it?
