Looking to contact a Supervisor/HR

Data Analyst here. I'm applying for refinancing and they are asking for my employer to fill out an employment verification form. For my other job, I gave my paystubs and tax forms which worked fine. Since we only get remittance documents for Telus, and I haven't been working long enough for my 1099, I need someone higher up to confirm my employment.

The last time I needed something like this, it was impossible to find the right email addresses, and even when I did send emails, they didn't really help. I also saw a post from 4 mo. ago on here that suggested contacting HR but didn't provide contact information or how to find HR.

Any guidance on who to contact/how to find HR info?
