Bonus training bug

I'm trying to complete the Pell Los executioner bonus training for the Cutthroat's scroll. I know you do not have to take any damage, which I haven't and it still does not drop the scroll wtf?

I start from the Linon mine outpost blade, kill the enemies till I reach the executioner and I Javelin him to death. I take no damage by him or before him and still the scroll does not drop.

I have restarted the game 3 times already.

Another bug is I can't pick up off the ground near the bridge in the first area a nightmare log and that area shows incomplete, even though I've progressed through the whole game.
