I need to transfer my retirement account out of Chase to avoid fees but don't know how

Hi - I just read Ramit Sethi's book IWTYTBR (yes I know I am late to the game). He made it very clear that fees are the end of you and I now need to transfer my retirement account out of Chase to Vanguard. Can someone tell me how I do it? Is it as simple as opening the new account and initiating a transfer? I have a lot of anxiety about these things, so any explanation is appreciated.
