Wedding surname

Hi guys, I have a question about taking surnames.

I know that typically, women take the man's surname, but I wanted to change that. I want to take her last or middle name. Is that allowed?

And another question: My middle name gives me the right to have an Italian passport ( I have had an Italian passport for many years). If I get rid of that middle name, would that be okay, in the sense of me still having the right to obtain, renew, and pass it on to my children?
