Looking for a Free Survey Form That’s Not Google Forms?

Problem/Goal:

I’m looking for an AI tool that can help me create a survey or poll form ideally something free and customizable for my business.

Context:

I need it for a blog post, so I’d love to be able to embed it directly on the page. I’m hoping for something a bit more flexible than Google Forms.

Previous Attempts:

I’ve used Blackbox AI before for generating code (like clickable buttons for my blog), but I haven’t tried it yet for building a full survey form. Curious if anyone has experience with that or any other tools (aside from: Google Form & Blackbox AI) please recommend!
