F18, possible adenoma or fibrolamellar hepatocarinoma.

Hi everyone, i’m 18F approx 220lbs 5’4, In Mid to Late 2019 I started birth control as I have PCOS and was experiencing bad menstrual bleeding, dark coarse hair etc and they really helped at the time for that. In May-June of 2022 I was experiencing small cramp like pains in the right side of my stomach under my breast which led me to getting an ultrasound, they found a 5.4cm x 4.4cm liver lesion most likely a hemangioma, they determined it wasn’t possible for it to be linked to the pain and for the next few years I got yearly follow ups with ultrasounds.

In June of 2023 I woke up to a sound and randomly touched behind my ear and there was a very large bump as hard as bone (my parents thought the sound I heard was a bone breaking or popping out of place and that the bump was it sticking out since it really felt like it was a bone.) there, my doctor thought it could be either a cyst or a swollen lymph node, I had a CT and ultrasound done which said it is most likely a swollen lymph node, at the time of the CT it measured 8 x 6mm, no further testing has been done but it has never really went down, it has times where it feels flat as it should and then a few days later its back to a hard bump, but they have said it looks benign.

Around this same time I had my yearly ultrasound on my liver, the lesion measured approximately 5.7 x 5.3cm.

In April-May of 2024 I started having severe “episodes” of pain in the right side of my stomach where the liver is, during my yearly ultrasound in June gallstones were found, I had my gallbladder out in October, I sometimes still experience bad pains in the same area but not as much as when I had my gallbladder, but I can say the worst pain i’ve had like that yet was after my gallbladder was out for a few months (around January-March of this year). During that ultrasound the lesion also measured 7.3 x 5.1 x 4.0cm, I also should mention the surgeon who removed my gallbladder did not see the lesion (It is in segment 8 of my liver).

I had a MRI with contrast done in early January as my surgeon spoke to a few other specialists more specialized in liver lesions and they thought I would be fine but just wanted to make sure of the size and everything, i’m not sure of the exact size but it had grown to nearly 8cm, the results were inconclusive but they said it is not a classic hemangioma as suspected on ultrasound, that they should first consider an adenoma, then a giant hemangioma and lastly fibrolamellar hepatocarcinoma, and suggested I have a labelled red blood cell scintigraphy, if that was inconclusive one with colloid sulfide to confirm an adenoma or if those both were inconclusive an MRI with an injection of hepatobiliary contrast agent (Multihance), the last test my doctor says he has never heard of so if anyone can explain that to me I would very much appreciate it.

In late February I went to a clinic to have my birth control changed as the cost was expensive and I couldn’t really afford it anymore, it was here they told me that its very possible my birth control is linked to the lesion and advised me to stop taking the pill and anything with estrogen in it, I stopped it on February 28th.

On April 11th I had blood work done focusing on my liver function, everything seemed normal.

I had the labelled red blood cell scintigraphy on April 28th which was inconclusive suggesting either an adenoma or fibrolamellar hepatocarcinoma, the next test I will have is the colloid sulfide one.

In all honesty, i’m just feeling very lost and scared, and I would really appreciate if anyone can give me more info on what the test my doctor says he’s never heard of is, I’ve heard that adenomas are more likely to turn cancerous if they’re larger than 5cm which is why they’re usually removed at that size while if thats what I have its nearly 8cm so i’m really scared of that.. and i’m terrified of fibrolamellar haptocarcinoma, I really want to know what my chances are at this point of this thing being benign and me being okay, thinking it was a hemangioma was like this safety net to me and now its gone and I’m just hoping for the best, and if anyone can give any helpful info on my situation, whats gonna happen, what my chances are, I would appreciate it a lot.
