FBR Application

Hi guys! Wanting to make sure everything is correct for my application, I'm applying through my grandma and have all the required documents but when it comes to my mum I just want to confirm what I need. My mum and dad were married and now divorced, my mum never took his name but has since legally changed her surname. Do I need her marriage certificate or just the name change document? Thank you!
