Mr. X wall destruction inconsistent?

On my first ever run of RE2R, the Tyrant gave me a good jumpscare by breaking through the wall near the interrogation/observation rooms. On all subsequent runs, I never saw that happen again. I honestly forgot that he could do it. But on my most recent run, he did it again. So I’m just wondering, what are the conditions for him doing that?
