Lalezari's Game of Chess

This is Sunday's post early. I won't be around this entire weekend.

Coming out of ESMO, just because the leadership team remains tightlipped, many are ridiculously thinking that CytoDyn went silent. Let's try to rewire a few of those laggard neural pathways. Come on now! Our thinking is important. Let's try to come together on this topic!

Let's assemble a hypothetical situation and call it the Art of the Deal.

CytoDyn's Leadership Team could have shaken hands with anybody. This means that anybody who had listened to and who actually heard what Dr. Pestell was putting down who, in addition, possessed the means to do something about it would have sat down and at least had a discussion with the team. Everything Pestell said was reinforced by Lalezari in follow up question and answer. The poster was the talk of the town.

Our friend u/KuneneRiver had described somewhere the MOA, just as clear as day, for really anybody with a 5th grade education to understand. KR explained that when these tumor cells begin upregulating PD-L1 on the surfaces of their cells, they become "invisible" to our immune system. Our, immune system Killer Cells pass right over them. But when a PD-1 inhibitor is introduced, these tumor cells all of a sudden become "visible" to our immune system, thereby allowing our Killer cells to devour them. In addition, in that same explanation, KR confirmed that our MSS mCRC clinical trial was delayed because of this recently discovered MOA of changing Cold Tumors to Hot ones. That the CRC trial should likely be reconfigured such that once these patients upregulate their PD-L1 on their CTCs to a medium or high level, then the option to switching from leronlimab to a physician chosen PD-1 inhibitor would be given to the patient. All of that is certainly to be discussed and likely already finalized by the time ESMO Barcelona 7/4/25 starts.

The highlight at ESMO Munich 5/15/25 was CytoDyn's presentation. All of BP was informed and notified of these 5 survivors. Could anyone attest to that statement? Feel free in the comment section. The Proof is in the pudding and those patients are alive and kicking. now going on 5 years following the initiation of leronlimab which was subsequently followed by a PD-1 blocker similar to Roche's Tecentriq. In the upcoming MSS mCRC ESMO, it won't be very much different with 4/6 alive and kicking who were compliant with a similar post leronlimab treatment protocol.

Remember those couple of pre-clinical studies, which, already may possibly be complete with results, therefore, soon to be released and disclosed. One study results the combination with Trodelvy; another study results the combination with Keytruda and a third study takes a deeper dive into turning Cold Tumors to Hot ones by chasing the leronlimab treatment immediately with a PD-1 blocker, such as Merck's Keytruda, or Roche's Tecentriq or GSK's Jemperli. In my previous post Building Good Things, I offered a possible purpose to these results:

"The (3) pre-clinical murine studies in mTNBC beef up a BTD application in mTNBC*. An understanding of how compatible and synergistic leronlimab is with Trodelvy and with Keytruda shall be understood and potentially included into the* application for mTNBC BTD*. The increased rates by which leronlimab upregulates the expression of PD-L1 on the Tumor's cell surface shall also be obtained, disclosed and included in the same* application for mTNBC BTD*."*

A few companies are attentive and astounded by what they saw at ESMO. Some were so proud, they may not have even showed. But, they are the ones who do their due diligence study in the background. Some attended but understood nada. Mid-level Medical teams? If they were able to comprehend, then, more than likely, most if not all were astounded; as you are probably aware, a majority of those who might attend such events remain clueless; I however am speaking about those who do understand, and if they understood, then, you have those who like to pretend they weren't impressed.

Then, there are some though, who do understand and who do not pretend. There are some who come, who listen, who make some quick calculations and who want to go straight to the negotiating table. There are some who can and do understand the obvious and clear power that Pestell packed into the presentation they saw. This was a Breast Cancer Conference and these persons understood Breast Cancer to which Pestell presented a promising Cure.

This was a Breast Cancer Conference. Now, doesn't G have a pretty big drug in this mTNBC space? Oh yeah, they have Trodelvy or SG. Wasn't it exactly that same drug which took the BTD away from leronlimab a couple of years back. Today, CytoDyn's own Phase 2 mTNBC trial blows the doors off that. In addition, the pre-clinical combination study of Trodelvy and leronlimab pitted against mTNBC is just about complete and the results are pending. G certainly was at ESMO and certainly attended our presentation.

Still in hypothesis mode, I would think the pre-clinical results are already out. Let's theorize that Dr. Lalezari and G have already spoken in regards to these results. We already know that leronlimab works fantastic with chemotherapy.

"In addition to everything it does as I described above in previous posts, leronlimab also sensitizes the cancer to the chemotherapy which the patient is receiving. It makes the cancer tumor more susceptible to the chemotherapy. That means the chemotherapy is better able to destroy the cancer when leronlimab is on board than when it is not on board*."*

And what is Trodelvy anyway? Is it not a targeted chemotherapy. When describing an ADC, Emma Walmsley put it like this: they are "missiles" which are targeted only for cancer when she describes ADCs antibody drug conjugates. G has recently been granted BTD using Trodelvy against extensive-stage small cell lung cancer (ES-SCLC) whose disease has progressed on or after platinum-based chemotherapy. I said in "Leader of The Pack":

"But what if the upcoming [pre-clinical] murine study results prove that the combination with leronlimab + SG greatly and significantly extends Overall Survivability and Progression Free Survival? and if hypothetically approved, would allow SG to be 1st in line in combo with leronlimab, in EVERY mTNBC patient, not just the ~33% who are PD-L1+ with CPS > 10? What if the leronlimab + SG combination results would allow SG to be used initially, as 1st line for every mTNBC patient*? What would G do if this could be ascertained?"*

G might have been at the table at ESMO with Dr. Lalezari. But, was there an individual there representing G who actually had a head on his shoulders? Given the special situation, of adding a PD-1 blocker to follow leronlimab therapy, once PD-L1 levels exceeded certain TBD mid and high level thresholds, so as to achieve a CURE, according to CytoDyn's recently disclosed model of turning Cold Tumors Hot, Trodelvy is not a necessary contributor to that treatment model in order to achieve such a CURE; however, a PD-1 blocker surely is necessary in that model. All of this to be included in BTD application.

However, the improvements uncovered by combination use with Trodelvy would certainly improve every patient's prognosis, especially for those who, for whatever reason, would choose not to chase leronlimab treatment with a PD-1 blocker, but rather to augment initial leronlimab treatment with SC, a targeted chemotherapy, also to be included in BTD application if seen fit.

Remember, Pestell was presenting. Pestell has probably already finished the GlioBlastoma Multiforme pre-clinical murine study. Remember, Pestell had good results in his initial GBM pre-clinical murine study he performed in the past. Also consider the fact that Trodelvy crosses the BBB.

"Trodelvy, through its payload SN-38, demonstrates the ability to cross the blood-brain barrier and reach therapeutically relevant concentrations in brain tumors and metastases, offering a potential treatment option for patients with CNS involvement."

It is not unreasonable to consider that Trodelvy and leronlimab could be combined against GBM tumors.

Hypothesizing, What if they did shake hands? Remember, Tyler Blok was present at ESMO.

"3) In my entire history of being in the cardiac space; watching very closely deals we made and getting acquired five times and acquiring other companies; never had we brought a lawyer into any of these meetings in the early phase of these discussions. Lawyers were usually available near the end for verbal communication/clarification before finalizing a written agreement. So when we find out Tyler was in Munich my radar was saying this agreement is in the final formality stage! Certainly, not the beginning stages!"

Can you even imagine it? Shaking hands with G? The same people who stole away our mTNBC BTD, who colluded to trash our HIV-MDR BLA and who ruined our COVID trial for thedeathisnear. Very fair and valid questions. Negotiate Peace? A way to forgive? A way forward? Please, don't everybody go run out screaming and panicking right now.

Look, CytoDyn's long base has been work hardened. We are a seasoned group of individuals, not constructed from sheep destined for the slaughter. I do have the answer to the question of whether Lalezari would cut a deal with CytoDyn's arch nemesis. Let's clear all the smoke and the mirrors.

Assuming that they met, assuming that they spoke, let's say they they even discussed a plan. Lalezari knows the entire past. Lalezari understands their ever lasting short position and their role in the entire Amarex debacle and knows everything from Sidley Austin.

Regardless of everything he is already aware of, what then, would Lalezari still offer to G? Does Lalezari remove his finger from the button that delivers retribution and revenge? Why would he do that? In return for what? Take a look at the nature of this man in his petition to the NIH. He learns who out there is really about Peace and who is about continuing the war. Their true colors shall be revealed. Lalezari has the cards and he keeps them close to the vest. He finds the Peace he seeks. He has the MOAB in his back pocket along with the codes which detonate it memorized.

Here is the part that is not so easily answered. CytoDyn is not stupid. We have Lalezari, Blok and Pestell all there at ESMO. Lalezari remembers absolutely everything. Between the three of them, they can quickly recall all the bloodshed spilled by the hands of both G and Amarex.

So, some of the top concerns, hypothetically, Lalezari could have brought up assuming a conversation did take place:

  • No more collusion, no more sabotage.
  • No more paid shorts.
  • No more partaking of joy when perfectly timed hit pieces are released.

Let's also make the assumption that the pre-clinical was already resulted. Then, Lalezari shared with G said results and got their blood circulating.

He would have showed them that the combination drug could let them to be indicated immediately, not 2nd or 3rd line. "Trodelvy is used later (2nd-line or beyond) in all mTNBC patients, providing an OS of 11.8 months and PFS of 4.8 months*, but still a significant benefit for those who progress after prior treatments."* Getting G to the front of the line would be speaking their language. This is what they want to see. This is what they want to hear. This is where they want to be. At the front of the line.

But, Lalezari knows this probably won't fly over that well with his own CytoDyn shareholder base. This base has the recollection of too many bad memories. We have witnessed way too much. Just look at the swordsman's bitterness. So, then, why is our guy, Dr. Lalezari even in the same room with G? And here comes my answer:

  1. Lalezari engages G to control, not to capitulate. This policy can be summed up in these words: Lalezari's Leverage Lord's over his Loyalty. Lalezari doesn't give away handouts. He lays down an ultimatum. Lalezari says, if you want a future in mTNBC, you need to play by my rules, otherwise, we'll wipe the floor with you if you don't.
  2. This hypothetical discussion could just have been bait, and not already celebratory. Was a deal made? Likely Not. Not yet at least. Lalezari certainly would have offered his intentions though. Conditional intentions of course. Any mess-ups on their part, would get the hammer to drop, and they wouldn't want that to happen.
  3. G's prior bad behavior certainly would not be rewarded without his terms being clearly stated, agreed upon and met. Lalezari does not capitulate. Too much is at stake. Lessons have already been learned in the past. He no longer is intimidated by decisive actions. This is his turf, as he owns the cards.
  4. Lalezari is willing to speak with G because in doing so, the main challenge would be indirectly addressed. The FDA. It becomes G's obligation to bring leronlimab to fruition because they have the means. Lalezari puts it into G's court to get this to approval. This is Lalezari's strategy.
  5. Peace between CytoDyn and its arch enemy comes out from such a Synergistic Deal. This would be Lalezari's signature move. The Art of The Deal. Maximum results with minimum blood shed. Their possible meeting together breaks down the built up animosity established over the past decade and brings about a way forward without either party looking weak.
  6. Partnership with said Arch enemy is actually possible, provided that our borders are protected, that our interests are protected. CytoDyn can use this partnership to get the job done, even if they spent the last 15 years in collusion and sabotage against us, because, if this gets leronlimab across the FDA's approval desk, why on Earth would this solution not be considered?

Lalezari is not making friends. He makes moves. Lalezari plays 4D chess. The chess board is being re-arranged because he holds the cards. He is out there brokering Peace between us. This is between G and CytoDyn. Think about it when you see Lalezari shaking hands with the wrong people. All the great ones in history did the same thing. Sometimes, to secure Peace, you have to meet the Monster head on and show him who is boss, and that my friends is the Art of The Deal.

I felt like I had to call this out. Raw and Unfiltered.
