How do I get approved for a medical marijuana card in TX? Do I qualify?

Someone told me that if I get a medical marijuana card, it will help me focus better in school and help with stress. I've been on medication for anxiety, depression and schizophrenia. I also had a prescription to help with diabetic neuropathy. I'm also in remission for AML Leukemia. Do I qualify? If so, how do I go about getting a medical card?
