Flooring question

New homeowners here and we are wanting to replace the flooring for out living room, kitchen and hallway. The sub flooring is plywood, would we need an underlayment? Or a vapor barrier? The floor isn't in a basement but above it. Also is a diagonal pattern a bad choice? Or should we stagger it? Thank you
