Theories and headcanons about Ironwood’s injury

As in the one that necessitated him becoming a cyborg, which I'm honestly kind of surprised we didn't learn anything about during volume 7. I've seen a theory that it involved, as he described it in volume three, a "heat of the moment" incident, but given his character arc since then I think it might have been an actual betrayal. Any thoughts?
